Here are all the forms you might need to fill out for Youth Ministry events! If you don't see a form you need, please contact Melissa to get a copy of the form. Also, all forms can be picked up at church under the youth bulletin board (which is by the Danbury entrance to the church).

General Youth

High School

There are no forms for high school at this point.

Appalachia Service Project

If you plan to come on the mission trip, you must turn in a permission slip, UMCWFB medical form (above), the ASP release form, and a copy of your medical insurance card.
